Finasteride (Propecia) was first introduced in 1989 as a treatment for benign prostate hyperplasia (BPH), a condition which is also androgen-dependent like androgenetic alopecia or pattern baldness — the reason finasteride has also been used in the management of androgenetic alopecia.

The androgen dihydrotestosterone (DHT) is the cause of AGA. Excessive DHT levels in the scalp block the blood flow to the hair follicles, causing malnutrition then miniaturization of the follicles. In effect, the follicles can no longer produce healthy hair.

Finasteride is a selective inhibitor of type II 5-alpha reductase, the enzyme responsible for the conversion of testosterone into DHT.
